Why Glucose Screening Matters
Between 24 and 28 weeks of pregnancy, your body is in the middle of a remarkable metabolic shift. The placenta produces hormones that make your cells slightly more resistant to insulin, which allows more glucose to stay in your bloodstream and cross to your baby. This is by design. Your body is prioritizing fuel delivery to a rapidly growing human.
For most people, the pancreas compensates by producing more insulin and blood sugar stays in a healthy range. But for roughly 6 to 9 percent of pregnancies, the compensation falls short and blood sugar climbs higher than it should. That is gestational diabetes, or GD.
Screening matters because GD rarely has obvious symptoms. Without testing, higher blood sugar can go unnoticed, and sustained high levels are associated with larger birth weight, increased risk of birth complications, and higher chances of the birthing person developing type 2 diabetes later in life. The Standard Glucose Challenge Test
The most common screening in the United States is the two-step process.
Step One: The One-Hour Screen
You drink a 50-gram glucose solution, often called "the glucola." It is a flat, intensely sweet orange (or lemon-lime) drink that you finish within five minutes. One hour later, your blood is drawn. If your blood sugar falls below the cutoff (usually 130 or 140 mg/dL, depending on your provider), you pass and no further testing is needed.
Step Two: The Three-Hour Diagnostic
If your one-hour result comes back high, you move to the three-hour glucose tolerance test. This one requires fasting overnight. You drink a 100-gram glucose solution, and your blood is drawn at fasting, one hour, two hours, and three hours. A GD diagnosis typically requires two or more values above the thresholds.
Some providers use a one-step approach instead, going straight to a 75-gram, two-hour fasting test. This is more common outside the United States and is endorsed by the WHO and the International Association of Diabetes and Pregnancy Study Groups.
Why Some People React Poorly to the Glucola
The glucola drink works fine for many people, but others experience nausea, vomiting, headaches, shakiness, or a dramatic energy crash. The reaction is not surprising: 50 grams of pure glucose on a mostly empty stomach is a shock to the system, especially during pregnancy when nausea sensitivity is already heightened.
Vomiting during the test can invalidate results, which means repeating the entire process. For people with a history of hyperemesis, sugar sensitivity, or reactive hypoglycemia, the standard drink can be genuinely miserable.
It is worth knowing that the glucola is a screening tool, not a sacred ritual. The goal is to measure how your body handles a glucose load. The drink is simply one standardized way to deliver that load.
Alternative Screening Options
The Fresh Test
The Fresh Test is an FDA-cleared alternative to the traditional glucola. It is a fruit-flavored organic drink that contains the same 50 grams of glucose required for accurate screening but uses ingredients you can actually pronounce. Many people find it significantly easier to tolerate. It comes in several flavors and can be purchased online or through your provider's office. Because it delivers the same glucose load, it works with the standard lab protocol, so no adjustments are needed on the testing side.
Standardized Meal Alternative
Some providers allow a food-based glucose challenge. You eat a specific meal (often pancakes, juice, and syrup totaling approximately 50 grams of simple carbohydrates) and test one hour later. Research on this approach is limited compared to the glucola, but a handful of studies suggest it produces comparable screening accuracy. The key is standardization: the meal needs to deliver a known amount of glucose to keep results meaningful.
CGM-Based Monitoring
A growing number of providers are open to using continuous glucose monitors (CGMs) as part of pregnancy screening. A CGM is a small sensor, usually worn on the back of the arm, that reads interstitial glucose levels every few minutes and transmits data to your phone. Rather than a single snapshot in time, a CGM gives you and your provider a full picture of your glucose patterns over days or weeks. This approach is still considered off-label for GD screening, but the data it provides can be more informative than a single fasting test.
How to Discuss Alternatives With Your Provider
If the standard glucola does not feel right for you, the conversation is worth having. A few tips:
- Lead with curiosity, not confrontation. Try something like, "I have been reading about glucose screening options and would love to hear your thoughts on alternatives."
- Share your reasons. Whether it is a history of vomiting during previous tests, concerns about artificial dyes, or a preference for a food-based approach, your reasons are valid.

What a GD Diagnosis Actually Means
If you are diagnosed with gestational diabetes, take a breath. This is not a failure of willpower, discipline, or "eating too much sugar." GD is driven by placental hormones and genetic predisposition. Thin, active people get it. People who eat beautifully get it. The diagnosis says something about your placenta, not about your character.
Here is what the path forward typically looks like:
- Dietary adjustments. Pairing carbohydrates with protein, fat, and fiber to slow glucose absorption. Many people are surprised by how much difference meal composition (rather than restriction) makes.
- Movement. A 15-minute walk after meals can noticeably lower post-meal glucose levels.
- Monitoring. Checking blood sugar several times a day using a finger-stick meter or a CGM.
- Medication if needed. Some people require insulin or oral medication, and that is okay. It is not a personal shortcoming. It is a physiological need.
The vast majority of GD resolves after delivery. With proper management, people with GD go on to have healthy, full-term births.
